Clinical condition
Recent stability, respiratory and cardiovascular function, neurological status, pain, risk of change during the journey.
OUR METHOD
Every transfer begins with a clinical and logistical assessment. The aim is not to medicalise more than necessary, but to plan the level of support the patient genuinely needs.
Known information is enough to open a request; missing details can be completed afterwards.
When needed and with the required consent, information may be completed with the local treating team.
Patient condition, suitability for transport, duration, environment, transitions and foreseeable needs.
The transport mode and clinical team are determined after case review; patients and families are not asked to determine the solution themselves.
Preparation, ground connections, care in transit and handover to the receiving team according to the agreed plan.
LevOne Transfer Acuity Framework (LTAF)
An internal framework that helps structure the assessment. It is neither a validated medical score nor an automated decision algorithm.
Recent stability, respiratory and cardiovascular function, neurological status, pain, risk of change during the journey.
Oxygen, infusions, ongoing treatment, monitoring frequency, foreseeable nursing or medical interventions.
Total duration, transitions, cabin altitude, available space, airline or operator requirements.
Confirmed receiving facility, handover, diversion or intermediate care options.
Clinical judgement and the requirements of the transfer remain decisive. No score alone selects the team.
